service request form

Request Forms are forms used by employers and employees in business organizations when making a formal request, each with their own specific reasons. You can use these forms to request for service, feedback, maintenance, absence, sponsorship, and many more.

An example of a Request Form is the Service Request Form in which a user requests for information or advice, or for a standard change, or for access to an IT service. These forms are used to provide service requests of a client to a provider to successfully fulfill these requests in time.

Here are a few reasons to separate your standard Service Request Forms from a break-and-fix type of incident ticket. This helps you in better organizing the service requests that are more important than the standard low-impact changes. The details include:

  • Easier for customers to submit tickets – If you combine your service request forms or system with a service request catalog or self-service portals, customers will be able to view and select the exact service that they need, pre-filling most of the information that is required on the Service Request form. By making forms submissions easier, it would also be easier for the customer.
  • Reduce unnecessary approvals – Standard IT changes that have already been approved can be included in service requests and this reduces the number of requests that will go to your change management workflow, which would otherwise be more complicated. This saves you time and resources as well.
  • Cleaner reporting – Whether you are reporting for continual improvement or for compliance purposes, keeping service requests and other service desk modules far from each other will keep your reports clean and organized. This allows your team to easily see how many standard requests you are receiving. This also gives your team an insight and an understanding into where to designate resources.

Maintenance Request Forms are another example of a Request Form. This form is used when charging an organization for a request for repairs and maintenance; in most cases, a housekeeping maintenance. Oftentimes, it is advised that residents should never attempt to do their own repairs.

How to Handle an Increasing Amount of Service Requests

Good customer service is all about handling all requests that meet with the customer’s expectations or more. It can always be resource intensive and costly to maintain a good service, but in order to handle requests in accordance with the customer’s expectations, here are a few questions you need to ask yourself with regard to prioritizing requests:

  • Is it the customer’s category the customer belongs to? Do VIP customers get higher attention?
  • Do you want to base it on how long the customer has waited for a response for that request?
  • Do you want to prioritize new requests coming in, before the existing requests?
  • Would you rather prioritize requests that have been sent over and over again?
  • How fast does a customer respond to certain requests? Is it a lesser priority if a customer responds two weeks later, after you sent out your mail?

The better you are able to exceed and meet a customer’s expectations and standards, the more likely it is that the customer will remain a happy customer. Plus, you get happy Service Request workers who can focus on doing their job.
