Information to Be Included on Medical Records Release Forms
- The name of the covered entity that will release the patient health information
- Patient’s personal information, such as his name, address, contact details, and Social Security number
- Information of the other party that the covered entity will be disclosing the patient’s health information to, or the receiver of the information
- The explicit authorization of the patient
- Specification of what kind of information is authorized to be disclosed, such as billing information or medical treatment information

- The term of the Release Form, which is the amount of time the Release Form will be valid
