Employment Forms are necessary forms that a newly hired employee needs to fill out and are necessary requirements based on both federal and state laws. Employment Forms are typically kept in specific locations in the human resources department where employees who need them can easily see them.

Employment forms are important because they aid in the documentation and the organization process of an employee’s dossier. Most employment forms are also mandated by laws and labor agencies and should therefore be completed and submitted for processing.
Medical Health Forms
Before or after an employee signs an employment contract, employers will usually require them to submit results for a Physical Examination.

If you are working as a freelancer or a contractor and you are your own boss, then you are technically self-employed. You will then need to use Self Employment Forms to register yourself as self-employed and to apply for the Self-Employment Assistance Program which offers unemployed or displaced workers unemployment benefits or allowances to help them while they are still starting out.

Form W-4 for Federal Income Tax Withholding
Newly hired employees need to complete a W-4 form prior to receiving their first paycheck wherein details on the employee’s marital status, number of dependents, and designated additional withholding amounts are being stated in the form. This form is updated according to the employee’s wishes. The employer should make sure that details on this form are up to date with the latest changes.
Form I-9 and E-Verify System for Employment Eligibility
The I-9 form helps employers document the eligibility of new employees to work in the US. Birth certificates or a green card must be provided to serve as proof of an employee’s eligibility.
Job Application Forms
One of the most common forms in the employment process is an Employment Application Form or an employment questionnaire form. An Employment Application Form is a form that requires applicants to provide details about themselves that are significantly relevant to their work or to the position that they are applying for such as the applicant’s name, age, date of birth, address, contact information, current and previous employers, educational background, and a list of character references.
While most employers are satisfied with the presentation of a resume or a curriculum vitae, other employers prefer to give out application forms as a means to protect themselves from hiring employees who are realistically unqualified for the position or from people who falsify their resumes. Having applicants fill out a job application form allows employers to ask further questions about an applicant’s identity and allows them to dig deeper into sections that show inconsistencies with the information written in a resume versus those that are indicated in a job application form.
